Reporting to the Director of Nursing, this position will manage nursing services employees, supportive health service activities, and community outreach-based nursing programming. This currently includes Transitional Care Management (Hospital-based and with Housing Intake & Placement Services); Street Health Outreach Program; and community-based nursing integration (Protective Action Respite, Residential Services, Family Support Services, Native American Services).

This position will also be responsible for personnel management, quality improvement, grant expansion, and program development for any future positions or programs that are related to this area of team expertise. This will be done in accordance with current federal, state, and local standards, guidelines, and regulations that govern CCH to ensure that the highest degree of quality care is provided. This position comprises of both administrative (75%) and skilled nursing care (25%).

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing andprivilegingtimeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  • Provides direct patient care. Maintains competence in all duties outlined in program(s)’ RN job descriptions.
  • Directly supervises Health Outreach RNs (DSOC & SOLE); HIPS RN; TCM RN; Protective Action RNs; and clinical volunteer staff associated with programs. With programmatic growth, will also supervise RN Coordinators, MAs, EMTs, LPNs, or PNs directly connected to community outreach programs.
  • Collaborates with stakeholders regarding patient care, health outcomes, programmatic growth and deliverables, integrated model and systems of care, as a both a leader in this sub-department and a representative of CCH. In this role, essential parties often will include but are not limited to:
  • a. People experiencing homelessness
  • b.CCH Integrated Healthcare Leadership & Interdisciplinary Team Members
  • c. Nursing students, medical residents, and university faculty; Denver-based hospitals; Denver shelter partners; Safe Outdoor Space staff, Denver Street Outreach Collaborative; St. Francis & Urban Peak Outreach Departments; HMIS partner leaders; Colorado Access; DDPHE; Parks & Recreation Services; City of Denver - HOST
  • d. CCH Housing Intake & Placement Service team; Housing First, SIB, & SIPPRA Teams; Residential Services; Family Support Services; Outreach Department
  • 4.Supervision:
  • a.Direct reports: Permanent, Temporary, Agency, & Volunteer staff in roles listed above
  • b.Performs or oversees various personnel actions related to hiring, orientation, onboarding, attendance, performance appraisals, and counseling
  • c.Holds regular 1:1supervision, providing feedback, coaching, mentorship, and support
  • d. For non-direct reports, professionally bring awareness of personnel issues to the attention of appropriate supervisors or HR for resolution
  • 5. Ensures high-quality, accessible delivery of care through Nursing Services:
  • a.Is responsible for ensuring the completion of competencies for all nursing and medical assistant staff members in sub-department; maintains high standard of care by offering or assigning relevant training and providing clear expectations of best practices
  • b. Ensures staff members are practicing within their perspective scope, at highest level, in accordance with the State of Colorado and organizational policies and procedures
  • c.Monitors section FTE and collaborates with nursing, operations, and integrated health leadership team members on staffing prioritization & decision-trees
  • d.Strives for excellence by leading staff and implementing policies, procedures, and programmatic strategies to meet national benchmarks or evidence-based best practices
  • e.Maintains quality assurance by maintaining or developing systems of monitoring, auditing, data tracking; evaluates & communicates with staff regarding quality assurance metrics; supports remediation, counseling, or positive feedback as needed

7. Effectively communicates with and to team members in a timely manner

  • a.Organizes and co-leads discipline meetings on a regular basis; maintains visual management board
  • b.Facilitates, leads, or participates in leadership or inter-disciplinaryteammeetings as appropriate
  • c.Cultivates an integrated environment for collaboration by ensuring communication among sub-department and program members is regular and routine through facilitation or promotion of focused team meetings, huddles, quality improvement events, and patient care conferences

8. Leads with quality improvement as a foundation to nursing and integrated health care

  • a. Proficient at Lean methodology within 6 months of assuming role
  • b.Coaches and holds standards for staff to implement and champion Lean initiatives
  • c.Collaborates on initiatives with leadership team members

9.Grounds program development within Healthcare for the Homeless foundational frameworks of care delivery, including but not limited to harm reduction; trauma-informed care; social determinants of health, person-first language; patient-centered care; integrated care models.

10. Develops or maintains necessary tools for data collection for purposes of quality assurance, quality improvement, grant reporting, and compliance. Files reports as required.

  • Ensures self, staff, documentation, and program services are within compliance of State, Federal, and HIPAA regulations. Endures adherence to infection control policies and Universal Precautions per CDC guidelines and OSHA regulations. Monitors and responds to occurrence reports when received.
  • Promotes integration and standardization of care throughout integrated health services across programs and sites as an ongoing part of our best practices policy.

Qualifications Summary

  • 1.Active Registered Nurse, licensed in the State of Colorado.
  • 2.Preferred two (2) years’ experience in a leadership role at a community health center, hospital, long term care facility, or other related health care facility; or, two (2) years’ direct care nursing experience at Colorado Coalition for the Homeless.
  • Preferred three (3) years’ experience as an RN.
  • Active Certification in Basic Life Support.

$86,781 - $104,138 a year
